ANY INSURED PERSON who desires to be attended after the 31st day of December , 1914 , by a Practitioner other than the Prac titioner who is attending him in the current year , or to obtain treatment by any of the methods referred to in the said Regulations , other than that which he adopted for the current year MUST GIVE NOTICE to the Committee before the 1st DAY OF DECEM- BER , 1914. SUCH NOTICE should be given , by sending to the Committee before the date named THEIR MEDICAL CARD together with a letter signed by the Insured Person stating his desire to change . ANY INSURED PERSON not making such appli- cation shall be deemed to have selected the Practitioner from whom he is now entitled to receive treatment , or to have applied to ob- tain treatment in the same manner as in the current year .
